Abstract

The sustainable appointment and dismissal of managers in healthcare systems is essential for maintaining stability, efficiency, and fairness in governance. This study examines the key dimensions that can strengthen leadership sustainability in national health systems and offers an integrative framework for evidence-based managerial processes.
A meta-synthesis approach based on Sandelowski and Barroso’s (2022) method was applied to systematically integrate qualitative and quantitative evidence. Comprehensive database searches combined with multi-stage screening ensured quality and relevance. Thirty-one credible studies were selected for in-depth synthesis.
Analysis revealed several interconnected dimensions crucial for sustainable management practices. Stability and continuity in leadership prevent operational disruption and maintain strategic direction. Meritocracy ensures appointments are based on expertise, scientific, managerial, and ethical competencies. Objective performance evaluation, supported by measurable indicators and periodic reviews, enhances accountability. Stakeholder participation fosters transparency and legitimacy by involving experts, NGOs, and professional associations. Equity in opportunities eliminates discriminatory practices and clarifies promotion pathways. Development of internal capabilities through training, succession planning, and talent management strengthens the leadership pipeline.
The proposed framework provides policymakers with actionable guidance for embedding sustainability principles into managerial appointment and dismissal processes. Implementing these dimensions can enhance governance, improve healthcare system performance, and build long-term public trust in leadership structures.
